Scottsdale – This week on FOXNews.com, I saw an interesting story about celebrities and their way-too-tight tops. The photos that accompany this article are quite revealing and I recommend that you see them for yourself. Fox claims these well-known starlets chose their dress size just a little too small and just tight enough for the breasts to really pop out.

According to Fox, this is a fashion faux pas. However, I believe not everyone is going to see it this way. I asked myself, isn’t this what all women who desire good cleavage want? Surely, this is why they invented the “push up” bra?

In my plastic surgery practice located in Scottsdale, Arizona I actually have patients who brought photos of some of the same starlets and said, “I want my breasts to look this way”.

Having a good size breast cup will definitely help to achieve a similar appearance. Breast augmentation and breast implants are only one way to achieve this look. Clearly, Fox demonstrates that a certain tightness on a bra or dress can increase just how revealing our ladies can be with their cleavage.

Eye popping claims are one thing, but I am looking forward to more articles of this nature. Ultimately, the famous people do seem to start certain trends when it comes to fashion and plastic surgery.

Flying off to an island for plastic surgery?

Scottsdale – Having a medical practice specializing in plastic surgery so close to the U.S.-Mexico border we see and hear about patients seeking plastic surgery south of the border.

There are two ways that we encounter patients after they have returned from their medical tour.

The first and most common, is when patients visit me at the AB Guerra Plastic Surgery Center, many months or years after the procedure. Patients consult with me at my Scottsdale, Arizona medical office because they are not completely satisfied with the results of surgery or because of complications from cosmetic surgery performed south of the border.

These patients often require revision plastic surgery, which can be quite costly. Any perceived benefits of financial savings from the original surgery are usually erased when a revision is required.

A second way we see patients who have had surgery is when they present with an immediate complication to an American hospital from the plastic surgery done outside the U.S. Fortunately, this is pretty uncommon. However, when this occurs, it is usually a far more serious situation.

The best way to prevent these unfortunate events is to educate patients about the differences between surgery performed in the U.S compared to surgery performed in other countries. The American Society of Plastic Surgeons has educated patients about medical tourism for many years.

In the U.S., plastic surgeons follow a culture of safety. In other words, we use the latest medical technology and knowledge in providing a reasonable standard of care, using evidence-based medicine. This standard is usually significantly better when compared to the medical standards in other countries.

We ask patients to consider that even routine post-surgery care, like changing dressings and monitoring healing can be necessary for several weeks. Most do not plan for that long of a medical vacation. Patients should also determine if insurance coverage for complications resulting from cosmetic surgery overseas will be covered. Patients should also realize that having the plastic surgeon of their choice, take care of a severe complication, on an emergency basis, may be a significant challenge.

Tips on plastic surgery before weddings.

Scottsdale – Plastic surgery for special occasions? I blogged about cosmetic surgery for the holidays last year.

With the wedding season fast approaching, I decided it was a good time to write about surgical and non-surgical options before the big day.

We classified cosmetic surgery procedures into those which are quick and easy to recover (hours & days) from,  and those which take longer to (weeks & months) recover from.

This information should be helpful for most people planning for that certain procedure. With this  information, you can plan ahead so you are fully recovered and ready on a particular date, for the big day.

The simple and easy: Botox, dermal fillers, and microdermabrasion.

We can classify Botox and dermal filler injections as the simplest and easiest of all the options. Many brides-to-be, mothers-of-the-bride and  (and fathers too), or just anyone that wants to look their best in front of family and friends should know about these non-surgical options.

These medical office procedures are performed quickly and recovery is just a few days. My advice is to have these types of injections 1 or 2 weeks before the event. To avoid bruising, you should not use Aspirin or other medications affecting your platelets before injections.

Botox Cosmetic and dermal fillers, like Restylane and Juvederm, are approved for the treatment of facial wrinkles. Botox is also approved for severe underarm sweating, which can be really helpful to any nervous bride or groom!

Facials and microdermabrasion, involving a spray of tiny crystals on the skin blasting away dead skin cells, reducing fine lines, brown spots, and small scars can be useful as a last minute enhancement for a wedding. Be careful if acne is an issue for you, because facials can sometimes trigger a breakout.

The just a little bit more involved: Skin care & IPL.

Laser therapy with intense pulse light photorejuvenation (IPL) performed a month before the big day can really help the facial skin to form a healthy glow for the wedding. IPL offers a quick recovery, but multiple treatments over several weeks are usually necessary to achieve the best results.

Some skin regiments may also take a few weeks to achieve the maximal effect. Obagi and prescription strength Retin-A may take up to 6 weeks before you start seeing smaller pores, smoother skin and a healthy glow. It is recommended you do not wax if you are taking Retin-A.

The longer recovery procedures: Facelift, Breast Augmentation, and Lipo.

Plastic surgery is real surgery and requires an appropriate amount of time for a full recovery. Many mothers and fathers-of-the-bride may choose eyelid surgery or even facelift to remove flabby skin in these areas. Eyelid surgery usually heals nicely by 6 weeks. A facelift may require a few weeks longer to recover. My advice is to give yourself plenty of time to recover from facial surgery.

Patients should realize, however, that with most plastic surgery procedures a full year is required before we consider you have reached your final result.

Breast Implants.

Breast augmentation is very popular before weddings. It takes about 6 weeks for most implants to settle into the right place. I recommend breast augmentation 3 to 4 months before weddings, to give your wedding dress maker time to size you up correctly. Once your breast implants have settled, it is fine to get measured.  Be sure there is enough time before the wedding to order your dress for a timely arrival.

Breast lift is popular too, but require just a few more weeks to be ready for wedding dress measurements.

Lipo for a wedding!

Liposuction is also very popular before weddings. The same issues with dress size measurements and wedding dress orders apply to those thinking about lipo before a wedding. Six weeks after lipo you will likely be ready to try on your new wedding dress. Liposuction recovery is tied to the amount of fat that is removed during the  procedure. The more lipo you have, the longer your recovery and the farther ahead you should plan for your procedure.

Tummy tuck before weddings?

We have seen brides, bridesmaids, and mothers-of-the-brides, get tummy tuck plastic surgery before weddings quite frequently. Other procedures like belt lipectomy, lower body lift, arm tuck and thigh lift are less popular before weddings.

Patients will really have to plan ahead before having one of the more involved procedures. For tummy tuck we recommend surgery at least two months before the planned wedding day. For belt lipectomy and thigh lift, we recommend, 3 to 4 months of recovery before the wedding date, might be better. An arm tuck should be performed 2 to 3 months before a wedding.

The final results of any surgery are not achieved until one full year after the plastic surgery date.

Scottsdale – Breast augmentation is the most popular plastic surgery procedure. What do you need to know when seeking bigger boobs?

We already discussed the option between silicone and saline breast implants. The next priority when seeking breast augmentation in Arizona, is the size of the new breast cup desired.

In our opinion, deciding on your new breast size after surgery is the most important decision when it comes to creating satisfaction. Fortunately, the final breast cup can be derived at, using common sense advice.

First, we must look at what the patient brings to the consultation. We must consider the height and weight of the patient. We also look at the proportions. We must consider the current bra size and chest measurements. Patients who are taller and wider can accept and look great with bigger implants. The more petite the person, the more difficult it becomes for them to accept bigger implants.

Second, we must consider what looks good on your body.  This is determined by two important factors. The first, has to do with your actual physical proportions. The second factor is more dependent on society, where you live and what sizes are considered acceptable. For instance, living in Scottsdale, patients spent plenty of time outside in the sun and larger breasts implants are more acceptable. An average size in the warm and sunny southwest, is between 385 to 465 cc.

The situation might be different if you live on the east coast. Patients usually choose smaller implants in this part of the country because outdoor activities are not as popular. Implant volumes less than 325 cc are more popular in this region.

Third, having a consultation with a board certified plastic surgeon, is one of the most important decisions you can make. We will encourage you to choose the size you believe will make you the happiest, but review the pros and cons of that size. When you are in our office, you will have plenty of time to tell us your desired cup and when you leave, you will be very confident about the size you have chosen.

In general, breast implants larger  than 350 cc are considered to carry higher risks leading to skin stretching, thinning and sagging. The larger the implant, the longer the recovery and there may be more pain for patients who choose larger implants. Large saline breast implants are likely to create a higher risk of rippling.

The bigger the breast implants, the more likely your result will look round, bubbly or fake. A natural result is more likely to be achieved when the implant size is proportional to your body.

38 KKK: That’s right, the biggest breast implants can be found in Texas.

Scottsdale – According to the NY Daily News, Texas woman, Sheyla Hershey,  has the world’s biggest breasts, at 38 KKK. Her story has been documented by several media outlets. In June 2008, she was a 34 FFF and was denied additional breast augmentation surgery by American doctors. She decided to go to Brazil and had a ninth breast surgery.  Brazil does not have a limit on how large breast implants can go.

She now wears the 38 KKK.

“To me, big is beautiful. I don’t think I have anything to worry about,” Hershey told Fox News.

According to Fox News, she holds the world record for the biggest breast implants, having an entire gallon of silicone to accomplish her new size. You can see more photos and read more about Ms. Hershey on MySpace.

IMCAS: Big aesthetic meeting brings good news.

Scottsdale – The International Master Course on Aging Skin (IMCAS) is being held this week in Paris, France. This is one of the biggest aesthetic meetings in the world and important economic news are coming from the presentation platform. According to the international experts, beauty therapies including laser treatments, breast enlargements and wrinkle-filling injections should grow 5 to 10 percent a year worldwide until 2013 (Source: Reuters).

The financial crisis had a significant effect on the number of aesthetic surgeries performed in 2009, with a drop of about 15% on average. In Europe, revenues in the medical aesthetic industry fell 15 percent to 3 billion euros (4.30 billion dollars) due to fewer surgical procedures and a 40 percent decrease in energy-based therapy equipments like ultrasound massages and laser treatments.

“Among patients it’s increasingly fashionable to want to keep one’s facial expressions intact rather than opt for a more drastic change in looks through surgery,” said plastic surgeon and IMCAS director Benjamin Ascher. “Medicalized beauty treatments can no longer be seen as a pure luxury,” Laurent Brones, business development manager at Symatese Biomaterials, a French maker of collagen.

Medical spas still have ample room for development in Europe. “Europe lags behind compared to the United States,” Dr. Ascher said. “The potential is huge.” According to this article, revenues from breast surgery should recover again, growing between 5 and 10 percent annually through 2013, after a decline of 15 percent last year.

Women represented 91 percent of plastic surgery patients in 2008, according to the American Society of Aesthetic Plastic Surgeons. IMCAS meets every year in Paris, gathering aesthetic surgeons and dermatologists. The four-day conference began Friday.

Yogurt as a simple way to reduce antibiotic related diarrhea?

Scottsdale – Patients undergoing elective plastic surgery have to use antibiotics for prophylaxis against infection. One well-known side effect of all antibiotics includes antibiotic induced diarrhea. In fact, antibiotic-associated diarrhea is the most common adverse effect of antibiotic therapy. It would be nice to have a means to avoid this uncomfortable and potentially dangerous complication.

Well, some research has demonstrated that eating yogurt before, during and after the episode of antibiotic use could reduce and/or minimize the length of symptoms. A study published in the journal Digestive Diseases and Sciences, included 200 hospitalized patients receiving antibiotics which were randomized to receive or not receive a commercial dietary yogurt supplement, and followed for 8 days.

The research was performed through the Department of Medicine, at Indiana University School of Medicine and the Roudebush VA Medical Center. The aim of the study was to determine the effectiveness of a dietary supplement of yogurt for prevention of antibiotic associated diarrhea.  In the research, the mean age of patients was 70 years and 43% were male. The compliance with the 8-day regiment was 85% and the with the study follow-up, 91%, respectively.

The results demonstrate that patients receiving yogurt reported less frequent diarrhea (12% vs 24%), which is a significant difference. Patients receiving yogurt also demonstrated significantly less total diarrhea] days (23 vs 60). The conclusion of the research demonstrated that dietary supplementation with yogurt is a simple, effective, and safe treatment that decreases the incidence and duration of AAD.

Not everyone agrees with these results, however. Similar research from the UK, demonstrated that probiotic yogurt did not make a difference in reducing diarrhea-related side effects. This research was from a single center and included 369 total patients.

To end the debate, we looked at another study from the Imperial College (Medicine) , London, using a probiotic containing drink. This research was published in the British Medical Journal, and showed that probiotics are effective in reducing the symptoms of antibiotic associated diarrhea.  The researchers concluded that consumption of a probiotic drink containing L. casei, L. bulgaricus, and S. thermophilus can reduce the incidence of antibiotic associated diarrhea and C difficile (the biggest culprit) associated diarrhea. The studies warn that if patients are immunocompromised, then probiotics may not be beneficial and could be dangerous.

Based on this review, it is my opinion that yogurt consumption, in healthy patients, receiving prophylactic antibiotics for elective plastic surgery, is safe and appropriate. We recommend patients begin yogurt 1 week before surgery and complete a 14-day, three yogurts per day regiment to minimize the risks of antibiotic-associated diarrhea.
