Nasal Surgery Book from 1597 Sold to Plastic Surgeon.
1Scottsdale – A recent report outlines how a rare jewel of a book has sold at auction and some lucky plastic surgeon has landed a piece of history. This book which details in highly illustrated diagrams the process of nasal restoration was sold for 11,000 pounds (or $17,000 dollars). This incredibly rare work is written in Latin and demonstrates how a patient’s nose attached to a flap of skin from the upper arm is performed.
This plastic surgery operation- known as rhinoplasty – shows the technique as it was used over 400 years ago. Nasal plastic surgery is far more common today and it is well known how celebrities undergo nose jobs for reasons of vanity. Of course, when this book was written, the operation was carried out to repair faces wounded in battle.
The book was sold to a modern-day plastic surgeon and was written by Gaspare Tagliacozzi. Tagliacozzi was a professor of surgery and anatomy at the University of Bologna and devised ways of repairing noses, ears and lips. In the photo above, he is shown holding some of his anatomy books. Many medical historians consider Tagliacozzi the “father” of modern plastic surgery. Of course, this almost didn’t happen. The book, which was published in 1597, was all but forgotten after the author’s death 2 years later. The technique was not rediscovered until the 18th century when the interest in human anatomy resurfaced once again.

Dermal fillers in rhinoplasty?
0Scottsdale – Dermal fillers are not FDA approved for rhinoplasty or post-rhinoplasty correction. However, because of their potential reversibility, low risks and ease of use, these are used in the nose with high frequency, especially for correction of minor defects after rhinoplasty.
The latest issue of the Aesthetic Surgery Journal discusses the use of dermal fillers to correct post-rhinoplasty deformities in the nose. The article was published through the University of Illinois Medical Center in Chicago.
The authors point out that dermal fillers are minimally invasive and can reduce the financial expense, anesthetic risks, or downtime associated with revision nose surgery. The researchers discussed these options and give some sound advice.
The authors like hyaluronic acid (HA) derivatives, such as Restylane and Juvederm, and calcium hydroxylapatite gel, such as Radiesse. Tips on patient selection and injection technique in the article are important, including:
1. Nasal injection includes sub-SMAS placement to reduce visible or palpable nodules.
2. Restricting the use of fillers to the nasal dorsum and sidewalls.
3. Avoiding injections to the tip and alae, which carry more adverse risks with injections.
4. Radiesse is an acceptable treatment for post-rhinoplasty deformities, but patients must be carefully selected to minimize risks.
5. Avoid silicone altogether because of severe granulomatous reactions, infection, thinning of the skin, and necrosis.
I found the article to be quite useful. I do agree with the authors, patients who are treated with dermal fillers must be followed closely for complications. The use of any soft tissue filler must be carefully considered by the patient. An explanation of the risks and the off-label use must be clearly explained to patients before any non-surgical plastic surgery procedure is administered (model used in illustration).
Some Thoughts on Rhinoplasty.

1. The goal of nasal surgery is to achieve a natural-looking nose as opposed to one that appears operated upon.
2. Each nose is shaped to fit in with the distinctive features of the patient’s face.
3. The shape of the nose should be improved, and importantly, the appearance of the entire face should be enhanced. There are limitations on what can be done.
4. Medical grade photographs will be taken to be analyzed in planning for surgery and to define the unique characteristics of your nose and face.
5. Dr. Guerra will discuss the steps involved in your rhinoplasty, using detailed diagrams which are easy to understand. When both parties agree on the plan, we will schedule surgery.
This discussion includes a disclosure of risks, benefits, imponderables and expected outcomes after surgery. There are no guarantees with rhinoplasty. Across the nation, re-operations from rhinoplasty are very common.

Aldo Benjamin Guerra, MD of Scottsdale, AZ to Present on Rhinoplasty and Abdominal Etching at National Meeting
0SCOTTSDALE, AZ – MAY 7, 2007 – Aldo Benjamin Guerra, MD, a local plastic surgeon in Arizona was notified today by the American Society of Plastic Surgeons that both papers he submitted have been accepted for presentation at the annual meeting held in October later this year. The first presentation summarizes Dr. Guerra’s experience with soft tissue augmentation in rhinoplasty over the past 24 months. His second presentation focuses on the use of liposuction for the enhancement of the abdominal muscles, a technique called “abdominal etching”, which Dr. Guerra has utilized for the past 5 years.
In the rhinoplasty presentation, Dr. Guerra plans to discuss the use of fascia, a soft tissue he obtains during surgery from the back of the patient’s ear to rebuild the nose. Dr. Guerra notices that some patients require augmentation as supposed to reduction of the nose to improve the appearance of the individual. Dr. Guerra stresses that he prefers a more natural result with all his procedures, but is especially interested in achieving the “most natural” results with nasal reshaping surgery.
“I have always had a great interest in nasal surgery and this paper describes a technique which I believe will assist many surgeons and patients alike to achieve a natural result with augmentation rhinoplasty,” said Dr. Aldo Guerra, Medical Director of the AB Guerra Plastic Surgery Clinic, Scottsdale, AZ.
In his second presentation, Dr. Aldo Guerra plans to discuss his experience with abdominal etching, a procedure used to enhance the appearance of the abdominal muscles. He plans to summarize, for the members of the society, the steps he takes to achieve an effective enhancement of the abdominal muscles while reducing potential side effects.
“Abdominal etching is a challenging procedure for most plastic surgeons. I believe that the steps outlined in my study will help surgeons and patients achieve better and more predictable results in the future,” said Dr. Aldo Guerra.
