What about breast implants? It turns out that there are numerous choices for breast implant shape, size and material. The two basic choices include saline-filled or silicone gel-filled breast implants.
Breast implants filled with saline are tried and true. They have been around a long time and can give you a successful result with your boob augmentation. Saline is the sterile salt water used commonly in every hospital. It is benign and will not harm your body. Saline breast implants, however, are not perfect. Saline breast implants in small-breasted women can lead to a cosmetic deficiency know as ripples. Rippling can occur with any type of implant but is far more common with saline filled implants.
Not the same breast implants your mother used to know? That’s right. The new silicone gel-filled breast implants are not the same the FDA banned in the past. Comparing the new FDA-approved implants to previous silicone breast implants is like comparing apples to oranges.
Clinical trials demonstrate the safety of the FDA-approved silicone gel-filled breast implants. There is no evidence that silicone causes autoimmune disease, cancer and other systemic problems. Silicone leakage is a problem, and the design of the new third-generation breast implants does a lot to minimize leakage.
First, the gel material is different. The new gels are cohesive not liquids. They act more like a solid rather than a liquid. The gel holds together uniformly, but has a natural feel that resembles actual breast tissue. Second, the outer shell that holds the silicone gel filler inside is thicker on the new FDA-approved breast implants. It is a three layered design that is durable, but feels very soft to the touch.
With the new technology is silicone leakage a thing of the past? Not exactly, but the six year follow up, on leakage, for Mentor’s MemoryGel is a low 1.1%.
In other words, 6 years after breast augmentation, only 1 out of 100 women with this type of silicone gel breast implant will have leakage of silicone. That’s very impressive. In fact, these fine results have never been replicated in the history of breasts augmentation and breast implants.
Breast augmentation is now safer and more effective than ever before. Besides the implant choices just discussed, there are many other important choices you need to make before choosing bigger boobs.

The best part of the medical conference, sponsored by Mentor Corporation, was learning about biofilms. It turns out that many of the accepted concepts on how bacteria live out their lives may not be totally accurate! We used to think as bacteria as loners who would in isolation cause infections.
The reality is that most bacteria form communities and communicate with others to form a successful living environment – called biofilm.

Pain Pumps reduce pain for breast augmentation patients – study shows
1Scottsdale – Last month we highlighted an article about pain pumps in breast reconstruction. This month our focus is breast augmentation and pain pumps. The May 2208 issue of the Aesthetic Surgery Journal presented a large (644 patients) study about breast augmentation and pain pumps – the effectiveness and safety.
For the study, patients were given a choice of intermittent bolus or continuous flow catheters for pain control. Prior to discharge, a local anesthetic was instilled by a nurse into catheters for those patients receiving bolus self-administration. Patients then instilled 1 to 2 additional doses of the agent during the first 24 hours and recorded pain scores in the evening and the following morning. For patients using continuous flow catheters, the local anesthetic was used to fill pumps that allowed up to 2 days of continuous flow at a rate of 2 mL (about 1/2 a tablespoon) per hour per side. These patients recorded pain at time intervals similar to those of patients using bolus self-administration.
Questionnaires were given to all patients to determine the extent of pain reduction following the instillation of local anesthesia. More than 200 complete questionnaires were gathered, and pain scores from each of these were analyzed to gauge the efficacy of the instillation regimen.
Impressively, 88 percent of patients who self-administered medication reported pain reduction on the evening of surgery, and 78% reported a reduction the following morning. Catheters were maintained for 1 day post-surgery in 544 patients and for 2 days by 100 patients. One patient developed an infection of unknown etiology that ultimately resulted in the loss of a unilateral implant.
The take home lesson for me on this study is that paim pump catheters work and extremely well in reducing pain for breast augmentation patients. Importantly, the study was performed in a prospective style, which adds strength to the conclusions obtained by the authors. In summary, indwelling catheters for the postoperative instillation of bupivacaine are safe and effective in the management of postoperative pain. Patients benefit from an additional form of analgesia, limited to the operative site, which is effective. Both continuous flow and intermittent bolus self-administration systems are effective and their patient ratings are indistinguishable.
MemoryGel Silicone Gel-Filled Implants – Safe and effective at 6 years
0Scottsdale – A newly published study in the international plastic surgery journal, Aesthetic Plastic Surgery, summarizes the up to date results of Core clinical studies for Mentor (MemoryGel) Implants. The results after 6 years are extremely favorable and support the safety and efficacy of silicone gel-filled implants.
The FDA gave approval to the Premarket Approval (PMA) applications for the round, cohesive, silicone gel-filled breast implants of Mentor (MemoryGel) and Allergan in November 2006. Since, the silicone devices have been widely available to plastic and other surgeons.
The study included one thousand and eight (1,008) female patients had data collected on 1,898 implants, and were enrolled at 48 sites. There were 556 patients enrolled for breast augmentation and 145 patients for breast augmentation – revision. There were 251 reconstructions and 60 reconstruction revisions in the study. The rupture rate, suspected and confirmed, for primary augmentation was 1.1% and that for primary reconstruction patients was 3.8%, which are excellent numbers for 6 years of follow-up.
Capsular contracture, the hardening of the breasts which can be seen in patients with breast implants, for clinically significant Baker III (visible deformity) or IV (visible deformity and pain) contractures for primary breast augmentation was 9.8% and for primary reconstruction was 13.7% at 6 years, which are also impressive numbers. The re-operation rate after primary augmentation and reconstruction was 19.4 and 33.9%, respectively. Removal and replacement occurred at a rate of 3.9% of primary augmentations and 10.4% of primary reconstructions.
The patients in the Core clinical studies will be followed through for 10 years, with comprehensive annual patient and physician-reported evaluations of safety and efficacy forthcoming in the future. Based on the results, the report demonstrates that Mentor’s MemoryGel Silicone Breast implants represent a safe and effective choice for women seeking breast augmentation or breast reconstruction following mastectomy.
