The Round Block Breast Lift: I.

Scottsdale – The round block was a revolutionary technique in plastic surgery of the breasts. In this section, we plan to review the basic principles behind Louis Benelli’s Round Block Technique. I had to do some extra research on the the procedure and the doctor using Google.fr, the French version of Google, so I could provide our followers the most up-to-date information available.

The medical article describing this operation was published in the Aesthetic Surgery Journal in 1990. Anyone can access the abstract and the first page of this article online free of charge.

The Round Block breast lift was a breakthrough because:

  • The round block uses dermo-dermic, glandulo-glandular, and glandulo-musculoperiosteal unions fixed definitively with permanent suture. In non-medical terms, the plastic surgeon uses the breast tissue, the deeper portions of the skin, and the deeper supportive connective tissue to manipulate the breast and improve the results of breast surgery.
  • A big advancement is the safe use of permanent sutures to manipulate the breast gland and to control the diameter of the areola.
  • The operation uses a crisscross breast lift or mastopexy technique, and also uses a circular permanent suture of woven nylon to create a peri-areolar circular dermo-dermic scar block.

At the time, the Round Block was advocated in breast surgery, including:

  1. Saggy or large breasts requiring breast lift or breast reduction.
  2. For small breasts requiring the use of breast implants. The round block technique allows easy access for the insertion of the silicone breast implant prosthesis and simultaneously corrects a mildly saggy breast.
  3. In cases of breast biopsy, the round block produces a discreet scar and an improved breast contour.

Understanding Capsular Contracture: Part II.

Scottsdale – The last time we discussed capsular contracture, we found there was a lot of interest on this subject. We are going to discuss more details about this condition that may affect up to 5 percent of women with breast implants.

In general, there is no way of telling who is going to develop capsular contracture, but several predisposing factors may increase your risk. The current scientific research on this condition implicates inflammatory conditions as the basic building block that leads to capsular contracture. In essence, any condition that leads to inflammation in and around breast implants can potentially lead to the formation of capsular contracture.

The goal of breast surgery is to place the breast implant into the appropriate location with minimal trauma and to reduce inflammation.

The factors that predispose to the formation of capsular contracture are discussed below:

  • Severe trauma to the breast. Any kind of trauma years and months later can affect the breasts. Reports of nipple piercings and car accidents, years after breast augmentation, leading to contracture are quite common.Trauma from surgery may lead to inflammation and increase the risks of contracture. An experienced, board certified plastic surgeon can perform breast augmentation while minimizing the amount of trauma to the breast itself. A clean and atraumatic separation of the muscles is what I prefer during breast augmentation surgery, always using a lighted retractor to see inside the pocket and gently separate the space for the breast implant.
  • Hematoma is a blood clot that is formed when there is bleeding into the pocket where the breast implants exists in your breasts. You should avoid blood thinners (coumadin, fish oil, vitamin E, ginko), platelet medications (aspirin, ibuprofen) before breast surgery and vigorous activity that raises your blood pressure and heart rate for a minimum of 2 weeks after breast implant surgery.   • Seroma is a collection of lymph fluid under the breast skin and surrounding the breast implant. Seroma formation is far more common in breast reconstruction, but can affect breast augmentation patients, in rare cases.
  • Bacterial infections can increase the risks of capsular contracture. Fortunately, frank infection is rare with the use of preoperative intravenous antibiotics. A more deceptive approach which bacteria take is the formation of biofilm communities which have been recently discovered. Double or triple antibiotic irrigation, nipple covers, and avoiding contact with the breast implant before insertion all reduce biofilm formation.
  • Silicone molecules leaching into the pocket surrounding the implant occurred quite commonly with old style silicone breast implants. The new cohesive gel, tripled-layered capsules silicone breast minimize this problem.
  • Breast implant placement above your muscle is more likely to lead to capsular contracture formation.
  • Autoimmune disorders are medial disorders that create inflammation in our bodies, potentially increasing the risk of contracture.
  • Smoking reduces the oxygen level in our blood and during healing. Less oxygen and the toxins in cigarette smoke combine to potentially increase the risks of contracture.
  • Radiation therapy used in the treatment of cancer can increase the risks of contracture when a woman undergoes breast reconstruction.

Not all forms of capsular contracture require treatment with Arizona plastic surgery. In part III of our capsular contracture series we will discuss what patients and doctors can do to reduce the risks of capsule contracture. There are four grades of breast capsular contracture, named, Baker grades I through IV.

The Baker grading is as follows:

  • Grade I - the breast is normally soft and looks natural.
  • Grade II - the breast is a little firm but looks normal.
  • Grade III - the breast is firm and looks abnormal.
  • Grade IV - the breast is hard, painful, and looks abnormal.

Treatment for capsular contracture will be discussed in part IV of our series.

Scottsdale – When we discuss breast augmentation with our plastic surgery patients, we discuss Capsular Contracture. What is capsular contracture?

A capsular contracture occurs when the lining of scar tissue which naturally forms around breast implants begins to tighten and becomes quite firm.

When saline or silicone breast implants are placed in the body, your body forms a lining around it. This phenomenon is normal and occurs with every type of implant which is placed in the human body. The lining or capsule is formed by your own living tissue.  Plastic surgeons refer to this lining as the “capsule”, “tissue capsule”, or “scar capsule”.

Capsule contracture is the most common complication of breast augmentation surgery. It can can happen at any time, but seems to be more common in the first several months after cosmetic surgery of the breasts.

At the time of the initial breast surgery, a pocket is made for the breast implant. During the healing process, a capsule forms. The body is genetically programmed to shrink scar tissue. Under normal conditions, the pocket remains open and soft, allowing the breasts to look and feel natural.

In some people, the capsule will tighten, and squeeze the breast implant. When the capsule tightens or contracts, the breast implants begin to feel hard. With enough pressure, the breasts appear distorted in appearance.

In the late stages of capsular contracture, the breasts feel very firm, and may take on a “ball-like” appearance. It is not the implant that has hardened. It is the shrinking of the capsule which compresses the breast implant that leads to the breast feeling firm and hard.

Scottsdale – According to a news report on Fox news, female suicide bombers are being fitted with exploding breast implants! Authorities say these may be impossible to detect – until it’s too late. MI5, the British spy agency, says they have also discovered extremists are inserting the explosives into the breasts during plastic surgery.

MI 5 believes the doctors have been trained at some of Britain’s leading teaching hospitals before returning to their own countries to perform the cosmetic surgery procedures. Top plastic surgeons confirmed the feasibility of the explosive implants. One claimed: “A properly inserted implant would be virtually impossible to detect by the usual airport scanning machines. You would need to subject a suspect to a sophisticated X-ray machine.”

Terrorist expert Joseph Farah claims: “Women suicide bombers recruited by al-Qaeda are known to have had the explosives inserted in their breasts under techniques similar to breast enhancing surgery.”

The lethal explosives called PETN are inserted inside plastic shapes during the operation, before the breast is then sewn up. The shocking new tactic involves radical doctors inserting the explosives in women’s breasts during plastic surgery. Explosive experts allegedly told MI5 that a sachet containing as little as five ounces of PETN could blow “a considerable hole” in an airline’s skin, causing it to crash.

“Given that the explosive would be inserted in a sealed plastic sachet, and would be a small amount, would make it all the more impossible to spot it with the usual body scanner.”

I was driving down Pueblo Street when I ran past the Santa Barbara Cottage Hospital. After a few blocks there is a collection of these flowers that my wife very much admired.
