Mentor Paragon: Breast Surgery.

Scottsdale – This week I had an opportunity to attend Mentor’s Paragon Symposium. The event was well attended by plastic surgeons from Phoenix and Scottsdale.
Mentor Corporation is the world’s biggest breast implant manufacturer and sponsors educational programs for physicians all over the world. The discussion Thursday night featured plastic surgeon, Dennis Hammond, MD from Grand Rapids, MI.
Dr. Hammond is a very well-known plastic surgeon, who specializes on breast reconstruction and cosmetic surgery of the breasts. He is best known for the SPAIR breast lift and reduction technique. He is also a consultant for Mentor and other companies which manufacture products for plastic surgery.
The topic for the night included the management of the breast crease and complications in cosmetic breast surgery. It was a very interesting lecture and discussion. The cases demonstrated varied from simple to very complex. For me, it was a pleasure to hear such a profoundly knowledgeable surgeon reveal some pearls about breast surgery.
The Round Block Breast Lift: I.

Scottsdale – The round block was a revolutionary technique in plastic surgery of the breasts. In this section, we plan to review the basic principles behind Louis Benelli’s Round Block Technique. I had to do some extra research on the the procedure and the doctor using Google.fr, the French version of Google, so I could provide our followers the most up-to-date information available.
The medical article describing this operation was published in the Aesthetic Surgery Journal in 1990. Anyone can access the abstract and the first page of this article online free of charge.
The Round Block breast lift was a breakthrough because:
- The round block uses dermo-dermic, glandulo-glandular, and glandulo-musculoperiosteal unions fixed definitively with permanent suture. In non-medical terms, the plastic surgeon uses the breast tissue, the deeper portions of the skin, and the deeper supportive connective tissue to manipulate the breast and improve the results of breast surgery.
- A big advancement is the safe use of permanent sutures to manipulate the breast gland and to control the diameter of the areola.
- The operation uses a crisscross breast lift or mastopexy technique, and also uses a circular permanent suture of woven nylon to create a peri-areolar circular dermo-dermic scar block.
At the time, the Round Block was advocated in breast surgery, including:
- Saggy or large breasts requiring breast lift or breast reduction.
- For small breasts requiring the use of breast implants. The round block technique allows easy access for the insertion of the silicone breast implant prosthesis and simultaneously corrects a mildly saggy breast.
- In cases of breast biopsy, the round block produces a discreet scar and an improved breast contour.

Seeking Bigger Boobs - Part 2
Scottsdale – Breast augmentation is the most popular plastic surgery procedure. What do you need to know when seeking bigger boobs?
We already discussed the option between silicone and saline breast implants. The next priority when seeking breast augmentation in Arizona, is the size of the new breast cup desired.
In our opinion, deciding on your new breast size after surgery is the most important decision when it comes to creating satisfaction. Fortunately, the final breast cup can be derived at, using common sense advice.
First, we must look at what the patient brings to the consultation. We must consider the height and weight of the patient. We also look at the proportions. We must consider the current bra size and chest measurements. Patients who are taller and wider can accept and look great with bigger implants. The more petite the person, the more difficult it becomes for them to accept bigger implants.
Second, we must consider what looks good on your body. This is determined by two important factors. The first, has to do with your actual physical proportions. The second factor is more dependent on society, where you live and what sizes are considered acceptable. For instance, living in Scottsdale, patients spent plenty of time outside in the sun and larger breasts implants are more acceptable. An average size in the warm and sunny southwest, is between 385 to 465 cc.
The situation might be different if you live on the east coast. Patients usually choose smaller implants in this part of the country because outdoor activities are not as popular. Implant volumes less than 325 cc are more popular in this region.
Third, having a consultation with a board certified plastic surgeon, is one of the most important decisions you can make. We will encourage you to choose the size you believe will make you the happiest, but review the pros and cons of that size. When you are in our office, you will have plenty of time to tell us your desired cup and when you leave, you will be very confident about the size you have chosen.
In general, breast implants larger than 350 cc are considered to carry higher risks leading to skin stretching, thinning and sagging. The larger the implant, the longer the recovery and there may be more pain for patients who choose larger implants. Large saline breast implants are likely to create a higher risk of rippling.
The bigger the breast implants, the more likely your result will look round, bubbly or fake. A natural result is more likely to be achieved when the implant size is proportional to your body.

Plastic Surgery after Major Weight Loss: Dermal Suspension Breast Lift.

Getting your new body after major weight loss.
Scottsdale – The popularity of gastric bypass and lap band surgery continue to rise. As a result, patients continue to seek plastic surgery after major weight loss. The breasts are one part of the anatomy which requires special attention in women after the weight comes off. Several techniques have been developed to deal with the breasts in these situations. However, very little has been published on the safety and efficacy of these techniques. For the blog, I am discussing an article which uses a innovative technique with very good results and low complications.
Breast deformities after weight loss vary significantly, but many patients report the classic changes with deflation of the breasts, medial localization of the nipple-areola complex and lateralization of the breast roll. The article published in Plastic and Reconstructive Surgery, reviews how the technique is performed, including the key steps, while summarizing the experience with this technique in 91 patients over the follow up period.
The technique is different from other breast lift techniques in that it uses several dermal flaps to raise the position of the entire breast gland and nipple complex. In fact, the central gland is elevated first and sutured higher via the dermal flap to the lining of the 2nd rib in the center. Then a medial and lateral flap is elevated and used to give the breast a round shape. Extensive dermal suturing is performed to reshape the gland even more to establish the desired shape. The standard anchor pattern is used for the surgery.
I have used this technique since it was originally published in a previous article, but was glad to see a well performed review. The breast related complications were only eight and included seroma (3), wound separation (3) and skin flap necrosis (2) of the T junction. In this series, most of the breast patients had other procedures performed including body lift, thigh lifts and arm tucks.
The before and after photos in the article are impressive and demonstrate that the upper pole can be maintained over the length of the follow up period. The authors concluded that implants are not necessary to achieve a good breast shape and nice upper pole volume in patients after major weight loss. On the other hand, 17 additional patients had breast lift with implant placement. This second group of patients had a statistically lower body mass index at the time of surgery. This suggests that, in this second group, the breast were more emptied out and required volume addition via the placement of the implant.
In my opinion, dermal suspension is a useful technique that is ideal for treating major weight loss patients which can be carried out safely with low complication rates.
