Primer on Buttock Shapes: Part II.
0Scottsdale – It is very important to understand the shape of the buttocks before performing buttock enhancement surgery. In fact, when planning a Brazilian butt lift, for instance, much of the fat in the back and hips is utilized for fat injections to the buttocks.
Brazilian Butt Lift and the V Shape Buttocks.
The goal with Brazilian butt lift is to improve the appearance of the back and butt. In many ways, the plan is to change the current shape of the buttocks from a V or square to a more attractive A and round shape.
The Square Butt Shape and Buttock Plastic Surgery.
In terms of overall results, it is the square shape that is the most challenging to change. The square buttock patient usually has extremely inelastic skin and little surrounding fat pockets. Additionally, the buttocks are pretty small and poorly defined. The buttocks are classically square and flat, on top of these other unfavorable characteristics.
The A Shape Butt: An Ideal Result for Plastic Surgery of the Buttocks.
The A Shape butt is the most attractive. If you consider that the top of the A is made up in the mid to lower back, then it is easy to see that removal of fat in this area will lead the way to achieving the most aesthetic result. The mid and lower sections of the A are achieved by adding volume and expanding the buttocks. People with A shapes have better buttocks when the bottom is very round.
The Round Shape Butt and Liposuction.
Since the round shape is considered the second most attractive, the plastic surgeon makes the best effort possible to round-up the buttocks even more. How is this done? There is a two-step process which involves liposuction in the lower back and hips with fat injections to the buttocks. The round buttock patient is very close to ideal and the results of fat injection with lipo are excellent.
Buttock Fat Transfer Resorption.
In general, the closer you are to the A shape before surgery, the most likely it is that your results will look far more attractive when compared to someone who has a square or V shaped buttocks. This fact has to do with the amount of change that can be accomplished during one operation. When we perform fat transfer now, we place a lot more fat than in the past, and the technique has advanced even more in terms of better equipment and technique. Additionally, we know that about 70% of the fat injected will survive and an over correction of 30% is planned for all buttock augmentation cases.
Silicone Buttock Implants versus Buttock Fat Grafting.
If you have a plastic surgery consultation in Scottsdale, Arizona and decide, based on our recommendations, that 700 cc of fat per butt cheek is what you desire, then the operative plan will include fat harvest of several thousand cc of fat and, at least, 1000 cc of fat injection is made into the each buttocks. When the anticipated 30% reabsorbs, then you still achieve the results desired with the original 700 cc plan. Buttock augmentation with fat grafting, in my opinion, is far better than using silicone buttock implants. The buttock implants are limited in size and are associated with all the limitations encountered with all other implants. The fat tissue we would use for your cosmetic surgery, on the other hand, all comes from your body, and there is usually plenty of it. Some patients may be too thin for fat grafting, but we can discuss other options.

Combined Medial and Lateral Thigh Lift with Buttock Augmentation.
0Scottsdale – There is a procedure that can lift the inner thigh, outer thigh and buttocks using a single operation. The procedure is called the Spiral Lift. The technique is interesting because it addresses these anatomic areas using a single incision. As a bonus, the tissue from the hips can be repositioned in order to augment the buttocks.
The article describing this operation was published in the journal Aesthetic plastic Surgery. The authors say that use of the technique in their practice was infrequent. Of 253 patients only 5 had the Spiral Lift. The results are indeed impressive, especially considering that surgery is carried out as an outpatient with a 23-hour stay, combined with an average of 5.3 liters of lipo and a relatively low blood loss of 220 ml. They report no major complications, re-operations and acceptably low complications. In fact, the patients only experienced minor complications.
The procedure is similar to circumferential body surgery, minus the anterior abdominal incision. This could be advantageous as the patient avoids an incision there. However, this could be considered a disadvantage if the patient requires abdominal skin resection or muscle tightening. The need for skin resection and muscle tightening in today’s society is very high. A bodylift would be more successful in these cases because it tightens the anterior abdominal wall, lifts the outer thighs, and buttocks.
Patients with the type of body contour deformity, the so-called “violin” shape, in which the Spiral Lift is used, are very rare indeed. The procedure might appeal to persons without abdominal excess skin or muscle laxity, but who may have saggy inner and outer thighs with small saggy buttocks. The authors should be commended for their ingenuity in the design and application of the Spiral Lift.
