Facelift Incision: Closure.

Scottsdale – When it comes to facelift incisions, we have reviewed our perspective on the anatomy of the Ear and the design of Facelift Incisions around these structures. We have learned a nice way to make the incisions less noticeable with a good design. To improve on this work, we should review the steps involved with facial plastic surgery wound closure. Creating a good closure can only improve the results of plastic surgery of the face in the hands of most board certified plastic surgeons.

How do we begin Closure of Facelift Incisions?

The closure of the face wound begins after the cosmetic surgeon has removed the skin judges to be excessive. The remaining tissue is closed under minimal tension. Once the tissues have been lined up, the closure begins with a deep layer suture technique. I prefer multiple, appropriately spaced 4-0 Vicryl sutures. This suture is placed up and down the incision, including the tragus, where a touch of cartilage may be incorporated to give the wound some strength. The same suture, which goes away over time, is used behind the ear.

What Suture for Facelift?

In front of the ear, for the second layer, we use a thin 5-0 non-absorbable nylon suture running up and down the incision, including the hairy scalp skin. I now avoid staples in the scalp. This was a fashionable way to close these wounds, but may lead to step-offs and less than ideal healing, in some patients. In the back of the ear, a suture that goes away is placed on the skin, including the hairy skin of the scalp there.

Easy Care and Good Results for Facelift.

This type of closure leads to the best results, in my opinion, but there are many respected plastic surgeons who may use a different technique. Another advantage of this facelift technique, is that it is easy to take care of until the sutures are removed by your cosmetic surgeon.

We look for excess fat pockets in the upper eyelids which contribute to the a heavy feeling before surgery. During eyelid lift, these fat pockets are removed. The incision is closed in the natural crease and results in a lift and tightening effect of the remaining eyelid skin.

The lower eyelid surgery compliments surgery of the rest of the face and upper eyelid lift. Our patients may experience a puffy appearance from the excess skin and fat bags making them look old and tired. Scottsdale, AZ – As an individual ages, the tone of the facial muscles decreases and the facial skin elasticity also diminishes. All these changes, allow wrinkles and lines to become more prominent.

Facial Aging and Plastic Surgery.

10 Tips for healing after facelift.

Scottsdale – These are our recovery tips for after the first week after facelift:

1. Mild bruising can be covered up. We carry a specially formulated brand of makeup for facelift surgery patients. You can begin applying this 7 days after surgery.

2. Numbness is normal. It is normal for sensory nerves of the face to have been disrupted during surgery. When applying make up, numbness in certain areas, close to incisions, may be noted.

3. Take it easy for several weeks after surgery. Do not bend over or lift heavy objects or run or do any activity that raises your blood pressure or heart rate (above 100) for 2 weeks after surgery.

4. High blood pressure is related to a higher risk of bleeding after facelift surgery. Do not participate in contact sports for at least 6 weeks. We recommend you avoid steam rooms, saunas, steam face masks, or any product that promotes flushing of the skin. You should avoid supplements containing niacin, which can also cause flushing until 12 weeks since surgery.

5. Avoid alcohol and products containing aspirin or ibuprofen for several weeks.

6. Smoking impairs your circulation and decreases your ability to heal. You should avoid smoking, first and second-hand, for as long as possible.

7. It is normal for you to notice a change in your smile, sensations of tightness, tingling, occasional sharp shooting pains, pulling, burning, and even cold sensations during the recovery period. These symptoms are usually mild and subside within the first 6 weeks.

8. A gentle massage with an Arnica-based ointment is extremely helpful in reducing these symptoms.

9. As the swelling subsides, a more youthful version of yourself will be revealed. This takes time and you should prepare yourself emotionally.

10. It is normal for some patients become depressed during the recovery period. Maintaining a positive attitude, eating a sensible diet, concentrating on re-establishing your normal routine, and beginning outdoor activities as soon as you feel up to it, will help you to get through the recovery period.

Aging in identical twins – factors contributing to an older appearance

Scottsdale, AZ - An interesting new study published in the international journal, Plastic and Reconstructive Surgery, involves sets of twins and makes an assessment of how environmental factors have affected their facial aging. In the study, 186 pairs of identical twins completed a comprehensive questionnaire, and digital images were obtained. An independent panel reviewed the images and recorded the perceived differences in the twins’ ages and their facial features. The perceived age differences were then correlated with multiple environmental factors.

The main purpose of this study was to identify the environmental factors that contribute to facial aging in identical twins, but it also confirms some of the observations noted by plastic surgeons over the years. The results confirm smoking and increased sun exposure  lead to significantly increased risks of aged appearance. In fact, the longer they smoked, the older they looked. Additionally, if patients did not use sunscreen and had a history of increased outdoor activity, then the aging appearance was accelerated. Wrinkles on the face were more common in twins who smoked or had a history of skin cancer (which implies more sun exposure). Dark and patchy skin discolorations were also more common in twins with a history of smoking and those with sun exposure.

Twins who used hormone replacement therapy had a statistically significant younger appearance. Dark blotches were seen less in those with a higher body mass index. Patients younger than 40 years with higher body mass index looked older, but the patients older than 40 years appear younger when they had a higher body mass index. The reviewers also noted that those who avoided alcohol looked significantly younger than the twin who consumed alcohol. Patients that had divorced or used antidepressants were also more likely to look significantly older.

Studies of this type confirm that smoking is not only bad for your health, but also for your looks. The study also implies that appropriate hormone replacement and nutrition may be significant in acheiving a more youthful appearance as individuals get older. I recommend avoidance of smoking and sun exposure to all patients contemplating plastic surgery. A good skin care regiment is also recommended for those who may want to reverse the aged appearance.  It is important to also recommend good nutritional habits and the avoidance of alcohol.
