New Bariatric Recipe: The Bear Claw Protein Shake!

Scottsdale – Hi, just tried this one out and it tastes great! I’m going to recommended to my plastic surgery patients. I got this recipe from the website, BariatricEating.com.

Here’s the Bear Claw Protein Shake they recommend:

  • 1 cup water
  • 1 1/2 scoops Lean Dessert Cinnamon Roll powder
  • 1 tablespoon Cinnamon Bliss or Caramel Sauce
  • 2 tablespoons sf almond syrup
  • 1 teaspoon sf lemon syrup OR drop of lemon extract OR 1/8 teaspoon lemon zest
  • 6 ice cubes

The instructions recommend you combine all ingredients in a blender and process.

For whom do I recommend this shake? I think this supplement good for any cosmetic surgery patient who has had bariatric surgery and is thinking about having plastic surgery after major weight loss.

When to start taking protein shakes before major body contouring surgery? My recommendations include the use protein supplements at least 4 weeks before lower bodylift surgery and continuing for a month after the plastic surgery operation.

Gastric bands to expand after FDA panel recommendatios?

Scottsdale – It is estimated, in a recent report, that about 12 million more obese Americans could soon qualify for bariatric surgery to implant a small, flexible stomach band designed to help them lose weight by limiting their food intake. The Food and Drug Administration will make a final decision on the Lap-Band in the coming months.

The Lap-Band from Allergan Inc. is currently implanted in about 100,000 people each year and usually helps patients lose 50 pounds. Under federal guidelines, it has been limited to patients who are morbidly obese. On Friday, an FDA panel recommended expanding use of the device to include patients who are less obese. The panel voted 8-2 that the benefits of broader approval outweighed the risks.

Experts stress the stomach band cannot stop deeply ingrained behavior that drives people to overeat. And the cost of the procedure will remain a barrier for many potential patients. The procedure’s costs can range from $14,000 to $20,000. The device itself costs $3,000.

More than a third of all American adults are obese. The gastric band was developed as an alternative to gastric bypass surgery, a permanent procedure in which food is rerouted from a pouch in the stomach to the small intestine. After major weight loss many of these patients may also have to see a plastic surgeon to deal with the excess skin that develops from weight loss. The recommended plastic surgery operations may include a tummy tuck or lower body lift, a thigh lift and breast surgery.

Any new indications approved by the FDA will also affect Ethicon Endo-Surgery’s recently approved  REALIZE™ Adjustable Gastric Band, a surgical implant for weight reduction and improvement in obesity-related health conditions.

Prineo: Initial Impression.

Scottsdale – I had the opportunity to use Prineo last week several times. Prineo is a new wound closure technique which combines Dermabond® and with a mesh tape. It is designed to replace the last row of sutures a plastic surgeon may place on incisions. The other name for this product is Dermabond® Protape. It as been used in Europe since 2007.

The product is distributed by Ethicon, which provides a multitude of surgery product lines including devices and products for plastic surgery after major weight loss, lap-band surgery, facial plastic surgery, etc..

Dermabond® uses the concept of “super glue” for medical applications and has been used in many surgical procedures. The medical glue has great adhesive properties, but is not perfect, and may not give the same results as a surgical closure. The glue alone is not considered to be as strong as a sutured skin closure.

With Prineo, the combination system of mesh and glue is three times stronger than glue alone and is considered to be the same as a standard suture closure.

I used it on buttock lifts, thigh lift, tummy tuck and lower bodylift procedures. The plastic surgeon starts by suturing closed the deep tissue layers of the incision. I also place a deep dermal closure with the Insorb® device. In some cases, I used the subcuticular suture while in others, I skipped the subcuticular portion and placed the Prineo technique. For accurate placement, the tape-like mesh is applied over the incision. The glue is lightly painted over the tape. Because the mesh contains the activating ingredient for the glue, is sets within a minute.

Having just used it this week and seeing the early progress, I was impressed by three aspects of the technique:

  • It is easy to use, it is fast, and efficient. This is truly an advancement in technique with time savings.
  • There is plenty of material to cover most plastic surgery operations with one device.
  • The strength of the closure is impressive and the mesh protects and covers the wound far more effectively.

Am I a Good Candidate for Lower Body Lift?

  1. lost a major amount of weight
  2. stable current weight for at least 1 year
  3. loose skin in their buttocks and back of their thighs
  4. loose skin in their hips and side of their thighs
  5. loose skin in the front and inner thighs
  6. loose abdominal skin
  7. relatively thin layers of fat below the skin
  8. acceptance of  permanent scars around the waistline

During consultation, your anatomy and proportions of loose skin vary will be examined.  The decision to utilize lower bodylift techniques depends upon your goals and expectations as well as an examination by your plastic surgeon.

Risks of Lower Body Lift.

SCOTTSDALE – No plastic surgery is without risks. Your plastic surgeon will explain the risks and benefits of your proposed surgery. This should help you with your decisions during the consultation.

Risks include blood clots, fluid collecting under the skin around stitches, nerve damage, infection, and healing problems. Your plastic surgeon will make sure you understand these risks, as well as the general risks with anesthesia required for your surgical procedure.

With a lower body lift, permanent scarring can be extensive. Your surgeon will inform you how much scarring you are likely to have. Many factors contribute to the depth, width, and color of scars.

There are no guarantees with what can be achieved with plastic surgery and patients with unrealistic expectations are usually unhappy with their plastic surgery results. Even when surgery is appropriately performed, patients may not be happy with their results, as there are simply too many limitations in shaping the human body for any surgeon to completely control the results.

Evidence-based Bodylift.

Scottsdale – The lower bodylift operation is an extensive plastic surgery procedure which carries a significant amount of risk. It is performed with increasing popularity in America because more patients are finding out about this surgery each and every day. The lower body lift or belt lipectomy is the most popular plastic surgery after major weight loss available.

The bodylift has several variations, which we will review in this blog. Most of the variants have to do with the location and design of the incision. In some cases, the incision location can be placed quite low. This hides the incision better and allows for a very nice upward pull on the thighs and buttocks. This gives the patient an attractive outer thigh lift, buttock lift while simultaneously improving the figure.

Another popular version places the bodylift incision in a higher location, just above the pelvic bones. These incisions allow for the most amount of tissue to be removed at the waist and can create a beautiful hour-glass shape. The incision may be more noticeable in such cases.

The third variation is often used in patients with vertical scars from open gastric bypass surgery. The design of this operation can include either a low or high waist incision and a vertical component where the healed bariatric surgery scar is located. This creates a T-incision right at the bottom of your tummy.

We have to consider when and how to best use these three options. The decision about these incisions should be carefully considered by each and every patient. Once a patient decides on their best choice of incision, the pros and cons are discussed once more with their board certified plastic surgeon.

The vertical component is especially discussed with this type of cosmetic surgery. This incision, if it is opened and re-closed, at the same time as a bodylift, could increase the risks of wound separation, infection, and delayed in wound healing. We know that studies demonstrate about a 50 percent chance of wound breakdown because of the length of the incision and relatively poor nutritional status after bypass while using a circumferential incision. What happens if we add an extra incision to our bodylifts? Such as the T-incision?

The most likely scenario is the risk of incision breakdown would be even higher than 50%. This is, of course, is problematic for both surgeon and patient. We have already reviewed on this blog about T-incisions performed for surgery after major weight loss. Thankfully, most wounds delays in healing and separations are relatively minor and heal quite quickly. Therefore, the key to the T -incision with bodylift or tummy tuck, is going to be the identification of patients which will benefit the most from this incision while minimizing the risks.

What’s the bottom line? I recommend the extension to the T-incision with bodylift when the patient will get a big improvement in the contour of the body. When the patient is borderline, the judgment to proceed with the vertical incision may be made intra-operatively. The plastic surgeon reserves the right not to proceed depending on various factors which can occur during surgery. We always keep the patient’s safety in mind and ultimate outcome when making these decisions.

I use this incision when the patient will get a better cosmetic result with a tighter tummy and a smoother waistline. I also recommend that patients increase their nutrition with vitamin C and a high protein diet for 1 month before surgery to optimize their condition. It is my opinion that when the surgeon, patient, surgery center and technique are all optimized, then we can achieve very nice results with minimal risks.
