Silicone Breast Implants.

Scottsdale – Many patients are contemplating a breast augmentation in Arizona and ask us about silicone breast implants. These questions are often about the differences between the new silicone breast implants when compared to the old breast implants.

We found a useful silicone breast implants video that you should watch:

The silicone breast implants video shows that after slicing through the breast implant shell there is no leaking of the silicone material. The Mentor silicone breast implants use cohesive polymers, not liquids, and it does not leak, as shown in the video. This means the gel filler acts as a solid rather than a liquid. Mentor gel breast implants hold together uniformly, while retaining the natural feel and appearance that resembles actual breast tissue.

In our Scottsdale plastic surgery practice, we use mostly Mentor silicone breast implants. Silicone gel filled breast implants have been approved for breast augmentation and breast reconstruction in the U.S. since November 2006. An article discussed previously on our blog, summarizes the findings involved with Memory Gel Core Study at 6 years. A close look at the breast augmentation subgroup demonstrates the 6-year rupture rate at a very low 1.1%. This is the lowest implant leakage rate ever reported for a long-term breast implant study.

Older silicone breast implants.

Older silicone breast implants were never FDA approved, did not have cohesive gel, and had weaker shells. Even when not ruptured, the older silicone breast implants, could leak their silicone into the body. The rupture rate for the older silicone breast implants was much higher than those established with the new FDA approved breast implants.

Two key points on the latest FDA-approved breast implants.

Let’s review two key points about how cohesive gel breast implants differ from older implants:

  1. Less seeping of silicone: Another key issue is the ability of the silicone particles to travel across the breast implant shell and into the body. With the Mentor cohesive silicone gel formulation the gel acts as a unit rather than a liquid, in other words, it holds together uniformly and retains the natural give that resembles actual breast tissue. It also helps prevent migration, or running, of the gel in the event of a rupture.
  2. A stronger shell helps: A key factor is the chemical make up of the implant shell wall. The strength of the material and optimal total thickness of the shell are important to reduce rupture and leaks. Mentor breast implants have a shell wall that is consistently strong with excellent impermeability, contributing to a very low leak rate of 1.1%.

Scottsdale – A new scientific study suggests that silicon found in beer, may be good for your bones.  According to this new information, beer is a significant source of dietary silicon, a key ingredient for increasing bone mineral density. It is proposed that moderate consumption may help fight osteoporosis.

The research was performed at the University of California-Davis and sought to determine the link between beer production techniques and the resulting silicon content.  The study tested 100 commercial beers for silicon content and categorized the data according to beer style and source. The researchers concluded that beer is a rich source of dietary silicon.

Based on these findings, some studies suggest moderate beer consumption may help fight osteoporosis, a disease of the skeletal system characterized by low bone mass and deterioration of bone tissue. “The factors in brewing that influence silicon levels in beer have not been extensively studied,” said Charles Bamforth, who led the study.

“We have examined a wide range of beer styles for their silicon content and have also studied the impact of raw materials and the brewing process on the quantities of silicon that enter wort and beer.”  Silicon is present in beer in the soluble form of orthosilicic acid (OSA), which yields 50 percent bio-availability, making beer a major contributor to silicon intake in the diet.

According to the National Institute of Health, dietary silicon, may be important for the growth and development of bone and connective tissue. The original article appears in the February issue of the Journal of the Science of Food and Agriculture.

I am happy to see this article, which places positive light on dietary silicon. Silicon is also used, as a basic building block, in the construction of many biologic implants, including silicone breast implants. The shell on the outside of saline breast implants is also derived from silicon. Dietary silicon is reportedly important in preventing osteoporosis.

Scottsdale – Breast augmentation is the most popular plastic surgery procedure. What do you need to know when seeking bigger boobs?

We already discussed the option between silicone and saline breast implants. The next priority when seeking breast augmentation in Arizona, is the size of the new breast cup desired.

In our opinion, deciding on your new breast size after surgery is the most important decision when it comes to creating satisfaction. Fortunately, the final breast cup can be derived at, using common sense advice.

First, we must look at what the patient brings to the consultation. We must consider the height and weight of the patient. We also look at the proportions. We must consider the current bra size and chest measurements. Patients who are taller and wider can accept and look great with bigger implants. The more petite the person, the more difficult it becomes for them to accept bigger implants.

Second, we must consider what looks good on your body.  This is determined by two important factors. The first, has to do with your actual physical proportions. The second factor is more dependent on society, where you live and what sizes are considered acceptable. For instance, living in Scottsdale, patients spent plenty of time outside in the sun and larger breasts implants are more acceptable. An average size in the warm and sunny southwest, is between 385 to 465 cc.

The situation might be different if you live on the east coast. Patients usually choose smaller implants in this part of the country because outdoor activities are not as popular. Implant volumes less than 325 cc are more popular in this region.

Third, having a consultation with a board certified plastic surgeon, is one of the most important decisions you can make. We will encourage you to choose the size you believe will make you the happiest, but review the pros and cons of that size. When you are in our office, you will have plenty of time to tell us your desired cup and when you leave, you will be very confident about the size you have chosen.

In general, breast implants larger  than 350 cc are considered to carry higher risks leading to skin stretching, thinning and sagging. The larger the implant, the longer the recovery and there may be more pain for patients who choose larger implants. Large saline breast implants are likely to create a higher risk of rippling.

The bigger the breast implants, the more likely your result will look round, bubbly or fake. A natural result is more likely to be achieved when the implant size is proportional to your body.

38 KKK: That's right, the biggest breast implants can be found in Texas.

Scottsdale – According to the NY Daily News, Texas woman, Sheyla Hershey,  has the world’s biggest breasts, at 38 KKK. Her story has been documented by several media outlets. In June 2008, she was a 34 FFF and was denied additional breast augmentation surgery by American doctors. She decided to go to Brazil and had a ninth breast surgery.  Brazil does not have a limit on how large breast implants can go.

She now wears the 38 KKK.

“To me, big is beautiful. I don’t think I have anything to worry about,” Hershey told Fox News.

MemoryGel Silicone Gel-Filled Implants – Safe and effective at 6 years

Scottsdale – A newly published study in the international plastic surgery journal, Aesthetic Plastic Surgery, summarizes the up to date results of Core clinical studies for Mentor (MemoryGel) Implants. The results after 6 years are extremely favorable and support the safety and efficacy of silicone gel-filled implants.

The FDA gave approval to the Premarket Approval (PMA) applications for the round, cohesive, silicone gel-filled breast implants of Mentor (MemoryGel) and Allergan in November 2006. Since, the silicone devices have been widely available to plastic and other surgeons.

The study included one thousand and eight (1,008) female patients had data collected on 1,898 implants, and were enrolled at 48 sites. There were 556 patients enrolled for breast augmentation and 145 patients for breast augmentation – revision. There were 251 reconstructions and 60 reconstruction revisions in the study. The rupture rate, suspected and confirmed, for primary augmentation was 1.1% and that for primary reconstruction patients was 3.8%, which are excellent numbers for 6 years of follow-up.

Capsular contracture, the hardening of the breasts which can be seen in patients with breast implants, for clinically significant Baker III (visible deformity) or IV (visible deformity and pain) contractures for primary breast augmentation was 9.8%  and for primary reconstruction was 13.7% at 6 years, which are also impressive numbers. The re-operation rate after primary augmentation and reconstruction was 19.4 and 33.9%, respectively. Removal and replacement occurred at a rate of 3.9% of primary augmentations and 10.4% of primary reconstructions.

The patients in the Core clinical studies will be followed through for 10 years, with comprehensive annual patient and physician-reported evaluations of safety and efficacy forthcoming in the future. Based on the results, the report demonstrates that Mentor’s MemoryGel Silicone Breast implants represent a safe and effective choice for women seeking breast augmentation or breast reconstruction following mastectomy.
