The seminar will be held in our office at the AB Guerra Plastic Surgery Center at 6 P.M. Last year, we held several seminars discussing the most popular topics in plastic surgery. Thigh Lift alters the Lymphatic System.
0Scottsdale – Patients after bariatric surgery for morbid obesity typically achieve a major weight loss. As a result, skin and soft tissue redundancy in some parts of the body, including the thighs, can result. The thigh lift is a plastic surgery operation that improves this redundant tissue in the medial thigh area, but the improvement can extend down to the knee in some patients which develop greater amounts of skin excess.
Common complications with thigh lift include inferior migration of the scar, delayed wound healing, vulvar distortion, and lymphatic system alterations. In my experience, thigh lift always results in temporary swelling in the ankles, legs and thighs. On occasion, I also have to deal with collections of undrained lymphatic fluid under the incisions, which may require external syringe drainage or catheter drainage. These collections are called seromas. Seromas are well known complications that occur with higher propensity in patients undergoing plastic surgery after major weight loss.
This prompted me to see if anyone had researched the effect of the thighplasty on the lymphatic system. I found a published study in Obesity Surgery journal (2008) that looked into this exact subject. This research involved 14 patients who were prospectively included in this study. Lymphoscintigraphy was performed before thighplasty, and this result was compared to a new examination performed 6 months after thighplasty.
Using a statistical analysis, the research study was performed on 26 inferior extremities. Patients with previous thigh lift surgery or abnormal scans were excluded from the study. All patients in the study group had normal lymphoscintigraphy before surgery. After thighplasty, eight patients (30.8%) demonstrated abnormal scans, a statistically significant finding. In the abnormal scans, significant functional and anatomic alterations in the lymphatic system were noted.
Review of this study supports my clinical observations that thigh lift surgery can alter the way lymphatic fluid drains from the lower extremity. The research demonstrates that thigh lift changes lymphatic drainage in lower extremities according to lymphoscintigraphy analysis in about 31 % of the patients. I suspect the actual number of altered lymphatics is higher, as some minor alterations, may not be picked up with this screening technique.

Patients commonly request thigh lift surgery. The thighs may easily accumulate fat and excessive skin in most people. With weight gain, the volume in the thighs is significant and the excess tissue may not disappear after weight loss. In fact, the thighs usually look worse with weight loss due to the accumulation of pockets of deflated skin and cellulite. Many patients, therefore, prioritize thigh surgery very high on their plastic surgery wish list.
What is a thigh lift?
A thigh lift most often refers to a procedure that removes unwanted excess skin, dimples and cellulite from the inner thighs. The procedure may be extended to include the entire thigh region from the groin and all the way down to the knees and beyond. An inner thigh lift is most often recommended after major weight loss to lift the saggy thighs and make the skin look smoother.

The steps involved in thigh lift surgery.
Surgery begins with appropriate markings performed before you go into the surgery suite. General anesthesia is provided safely by a board certified anesthesiologist. After anesthesia, the marked areas are incised and the excess skin is removed. The procedure allows for the maximal amount of tightening and size reduction. The final closure is completed with a carefully orchestrated multilayer technique and the incision is reinforced with a glue sealant that immediately and effective closes the wound, so that patients may safely shower soon after surgery. A light dressing is applied to minimize swelling and increase comfort.
Thigh lift goes smoother when performed at a hospital with overnight stay.
We prefer to perform inner thigh lift in a hospital setting and keep patients in the facility for several days. All patients are expected to walk the evening of surgery. The next morning, a soft diet is started after the patient gets out of bed and walks.Recovery from inner thigh lift usually takes about 6 weeks. Combined Medial and Lateral Thigh Lift with Buttock Augmentation.
0Scottsdale – There is a procedure that can lift the inner thigh, outer thigh and buttocks using a single operation. The procedure is called the Spiral Lift. The technique is interesting because it addresses these anatomic areas using a single incision. As a bonus, the tissue from the hips can be repositioned in order to augment the buttocks.
The article describing this operation was published in the journal Aesthetic plastic Surgery. The authors say that use of the technique in their practice was infrequent. Of 253 patients only 5 had the Spiral Lift. The results are indeed impressive, especially considering that surgery is carried out as an outpatient with a 23-hour stay, combined with an average of 5.3 liters of lipo and a relatively low blood loss of 220 ml. They report no major complications, re-operations and acceptably low complications. In fact, the patients only experienced minor complications.
The procedure is similar to circumferential body surgery, minus the anterior abdominal incision. This could be advantageous as the patient avoids an incision there. However, this could be considered a disadvantage if the patient requires abdominal skin resection or muscle tightening. The need for skin resection and muscle tightening in today’s society is very high. A bodylift would be more successful in these cases because it tightens the anterior abdominal wall, lifts the outer thighs, and buttocks.
Patients with the type of body contour deformity, the so-called “violin” shape, in which the Spiral Lift is used, are very rare indeed. The procedure might appeal to persons without abdominal excess skin or muscle laxity, but who may have saggy inner and outer thighs with small saggy buttocks. The authors should be commended for their ingenuity in the design and application of the Spiral Lift.
